1. Who we are and what this policy covers
Golfer’s Hub Pty Ltd (ABN 52 694 522 382), trading as Golfers Hub, operates from 24 Rocklea Drive, Port Melbourne VIC 3207.
Golfers Hub administers this website and the shared Golfers Hub Acuity booking and payment account. This policy applies to personal and health information Golfers Hub collects or holds through the website, general Hub enquiry form, Acuity booking and intake forms, email, phone, assessments and other direct contact with the Hub. It explains our practices under the Australian Privacy Principles and the Health Privacy Principles in Victoria where they apply.
The practitioner you select remains responsible for delivering their professional service and may keep a separate clinical, coaching or assessment record. A practitioner’s service-specific collection notice, consent process and privacy policy apply alongside this policy where relevant.
2. Information we collect
Depending on how you contact us, book or receive a service, we may collect:
- identity and contact details, including your name, date of birth, phone number, email, address, emergency contact and, for a junior, parent or guardian details;
- booking, attendance, cancellation, rescheduling, reminder and transaction records, including the selected service, practitioner, date, time and payment status;
- golf, training and performance information, including goals, experience, handicap, practice, coaching, competition demands and preferred starting provider;
- health and other sensitive information relevant to safe service delivery, including pain, injuries, medical conditions, medications, surgery history, pre-exercise screening responses and information needed for an emergency or referral;
- assessment and service information, including movement, strength, power, body-composition, biomechanics, images or video captured for analysis, recommendations and progress information;
- messages, consent, waiver, terms acceptance and optional media-permission records;
- the page used to submit the form and limited referral information, such as campaign or referral tags included in the page address; and
- if you accept optional analytics, page visits and non-sensitive actions such as booking-link, direction, phone, email, form-start, generic form-failure and successful-enquiry actions. We do not send enquiry contents, contact details or starting-guide answers to Google Analytics.
Please do not submit diagnoses, scan results, Medicare details or medical history through this general form. The Hub form is not a clinical intake form. Detailed information required for an osteopathy appointment is collected through Jon’s Acuity intake process after you select that service.
The Golfers Hub website does not directly collect complete payment-card details. Acuity/Squarespace and Stripe process booking and payment information when you make an appointment. Golfers Hub generally receives transaction status and limited payment information rather than your complete card number.
Images or video may be captured when they are needed for assessment, coaching or clinical purposes. Golfers Hub will not use identifiable images, video, testimonials or assessment material publicly for marketing without separate optional permission.
3. How we collect and hold information
We usually collect information directly from you or, for a junior, from a parent or guardian. This may happen when you submit the Hub form, book through Acuity, complete an intake form, screening form, waiver or terms, make a payment, attend an assessment or service, or contact us by email or phone.
With your authority, we may also receive relevant information from a practitioner, golf coach, school, parent, guardian or other representative. If you do not provide information required to identify you, manage a booking, screen for safety or deliver the selected service, we may be unable to respond, provide the service or may need to modify it.
Submitted Hub enquiries are stored in the website enquiry system and sent through our configured email-delivery service to the Hub inbox. Booking, intake and payment information is held through Acuity/Squarespace and Stripe. Selected practitioners may also record relevant information in their own professional systems. Hosting and security providers may process technical request information such as an IP address, browser type and request time to operate and protect the website. If you accept analytics, Google Analytics also processes limited device, browser, page and interaction information.
You can browse the site and use the three-question starting guide without identifying yourself. The guide’s answers exist only on the page for that visit. If you choose “Use my answers in an enquiry”, the selections are copied into the form for you to review or change. They are stored and sent to Golfers Hub only when you consent and submit the enquiry.
4. Why we use information
We use information to:
- respond to enquiries and identify an appropriate starting point;
- manage bookings, payments, reminders, cancellations, rescheduling and client support;
- screen for risk, manage safety and respond to emergencies;
- deliver, personalise and review assessments, treatment, coaching and facility services;
- prepare recommendations, track progress and communicate authorised findings;
- protect the website and meet legal, insurance, accounting, professional and business obligations; and
- where you have accepted optional analytics, understand which pages and non-sensitive actions are useful so we can improve the website; and
- send marketing or use identifiable media only where you have given separate permission or where otherwise permitted by law.
We do not use a general Hub enquiry as a clinical assessment, diagnosis or confirmed booking.
5. When information may be shared
We may share only the information reasonably required with:
- website, database, analytics, booking, payment and email-delivery providers that process information for us, including ChatGPT Sites (OpenAI), Cloudflare, Google Analytics, Acuity/Squarespace, Stripe and Resend, or FormSubmit if the notification configuration changes;
- assessment and coaching technology providers, including Sportsbox, Dēmōtu and InBody, where their systems are used for the selected service;
- the selected golf, performance or health practitioner for booking and service delivery, or another provider after you ask or agree that we pass on a general Hub enquiry;
- a golf coach, school, parent, guardian or other representative when you or an authorised representative have asked or agreed that we share relevant information;
- professional advisers, insurers or regulators where reasonably necessary; and
- emergency services or another party where required or authorised by law or necessary to respond to a serious threat.
We do not sell or rent personal information. We do not automatically send a general Hub enquiry to Fit To Play, Golf Performance Osteopathy or S.T. Golf.
6. Shared Acuity booking and payment system
Golfers Hub uses Acuity/Squarespace as the shared scheduling page for appointments with Adam Weeks, Jon Zuccala and Stuart Tennison. Appointment payments are processed through the Stripe account connected to Golfers Hub Acuity.
Your booking is assigned to the selected practitioner’s Acuity calendar. Golfers Hub administrators and authorised scheduling contributors may access information needed to administer the account. The selected practitioner may access the appointment, intake and assessment information needed to deliver their service. Access should be limited to the person’s role and the information reasonably required.
A practitioner may keep a separate professional record where required for clinical, coaching, insurance or legal purposes. Contact Golfers Hub or the named practitioner if you need to know who holds a particular record.
7. Overseas processing
Some technology providers used for website hosting, analytics, database infrastructure, booking, payment, email delivery and assessment technology operate in Australia and overseas. Google may process optional analytics information overseas. The United States is a likely overseas location for providers currently used by Golfers Hub. Provider support and subprocessor locations may change.
We take reasonable steps to use providers and handling arrangements appropriate to the information involved and to meet applicable Australian and Victorian privacy requirements. You can contact us for current information about the providers and likely processing locations used for a particular service.
8. Security and retention
We use reasonable administrative and technical safeguards to reduce the risk of misuse, loss, unauthorised access, modification or disclosure. No online service can guarantee absolute security.
General Hub enquiries are kept only for as long as reasonably needed to respond, coordinate the pathway and meet legitimate legal, insurance, security and business record requirements. When the information is no longer required, we take reasonable steps to securely delete or de-identify it.
Health records Golfers Hub creates or holds while providing a health service are generally retained for at least seven years after the last service, or for a person who was under 18 when the record was created until they reach 25, where Victorian health-record requirements apply. Booking, payment, accounting and insurance records may be kept for other legally required periods.
Booking and intake information may remain in Acuity for the applicable retention period. If information also becomes part of a practitioner’s separate clinical, assessment or coaching record, that practitioner’s retention obligations and privacy policy apply to their record.
9. Access, correction and consent
You may ask to access or correct personal or health information Golfers Hub holds about you. We may need to verify your identity and, if someone acts for you, their authority. We will respond without unreasonable delay and within the time required by law. Where the Victorian Health Records Act applies, the maximum period is generally 45 days for an access request and 30 days for a correction request. We will explain any lawful reason for refusing or limiting access.
You may withdraw optional marketing, media or information-sharing permission at any time. Media permission is not a condition of receiving a service. Withdrawal does not affect handling that has already occurred lawfully; we will stop new optional use and discuss removal of existing public material where reasonably practicable.
10. Children and young people
We take additional care with information about children and young people. A parent or legal guardian should complete bookings, safety information and consent for a child who cannot make an informed privacy decision themselves. We may also involve the young person according to their maturity and capacity. Junior participation is not conditional on optional marketing or media permission.
11. Website technologies and external links
Website infrastructure may use necessary technologies and process technical request information for delivery, reliability and security. Golfers Hub also provides optional Google Analytics 4 measurement, but it is inactive unless a valid Golfers Hub measurement configuration is enabled and you choose “Accept analytics”. No Google Analytics script, request or analytics cookie is initiated by this site before that choice.
If accepted, analytics records a sanitised page path and limited, non-sensitive action categories. It does not receive enquiry contents, names, email addresses, phone numbers, messages, health or body-status information, starting-guide answers, enquiry identifiers or raw page-query values. Advertising storage, Google Signals, ad-user data and ad personalisation are disabled in the site implementation.
Your choice is stored in your browser. You can accept, reject or change it using “Analytics preferences” in the footer. Rejecting analytics stops new measurement from this site and removes accessible Google Analytics cookies for the Golfers Hub domain.
Acuity and other linked or embedded provider, booking, payment, map and assessment services may use necessary cookies or similar technologies and are also governed by those organisations’ privacy practices.
